If your company has ISO (Internation Standard for Management Quality) you will need insert to your PO (Purchase Order) Form, this situation very simple step if you work with Crystal Report ( I am user CR XI) as attached pictures you can see Logo ( you can use any logo ) and inserted as OLE object in Crystal Report XI, from Design you can drag the object (logo) and edit it.
